I've noticed that there's an issue plotting curved polylines and I'm not sure what's causing it.
attached are the properties and a side by side comparison of what the model space looks like compared to the plotted pdf,
Why would it turn into a worm when i go to plot?
I also tested plotting a window from model space and it seems to be much less noticeable when it's taking up an entire letter or ledger sized page. It becomes noticeable again as soon as I turn off "fit to paper", adjust the scaling to 1:1 and zoom in on the preview pane.

Plotting normally uses drivers from plotter. If you, for example, try to plot in PDF - will it be the same?

That's how I primarily do my plots.
I'll plot to PDF (using the internal pdf Plotter) and print from the file. I've tested with other plot options as well and the only one that shows up differently is "Print as PDF"
Print as PDF appears to solve part of the issue, making it much less noticeable. There seem to be a bunch of other unrelated issues that I'd have to solve when using Print as PDF
My uneducated gut is telling me that this has something to do with how the thickness is being calculated and it's just really obvious on some of my prints for some reason.
The tests i just did using the default template file made it much less obvious.
(please see attached)

Hi,
this is a common issue while you printing polylines as a digital file format such as PDF or as raster image, the most common cause is the lineweight thikness.
To avoid this issue, you have to set the lineweight to 0.0, see attached files.
